School of Computing - Forms
- School of Computing Forms are accessible through the School of Computing Students course within Modules.
- For forms needing an instructor's signature, such as the Registration Waiver Request or Directed Independent Study forms, ensure you fill out and sign your portion before emailing the instructor for their signature.
- Completed forms should be submitted via email to computing@unf.edu.

Academic Plan of Study
- Form to assist students in designing a plan of study to fulfill their major requirements.
Probation Acknowledgement
- This form serves as your acknowledgment of probation status and consequences due to failing to maintain good academic standing or for violating the Satisfactory Progress Policy.
- This form, in addition to an updated Plan of Study will need to be signed before scheduling an appointment with your advisor.
Directed Independent Study
- Request permission to take a CIS4900 or CIS6900 Directed Independent Study course.
- Obtain your instructor's signature before submitting the form.
Computing CO-OP & Internship Agreement
- Request to register for CIS3949 or CIS5949 Experiential Studies in Computing. Read responsibilities carefully.
- Ensure you obtain the internship supervisor's signature before submitting the form.
- Please consult your advisor with any questions.
Registration Override Request
- Request to take a course without the appropriate prerequisites, or with time overlap with another course.
- Obtain your instructor's signature before submitting the form.
Return from Suspension Request

Oral Communications Skills Assessment
- This form is required to signify fulfillment of the Oral Communication requirement of all Computing majors. While most students complete this requirement in CIS3253, Legal and Ethical Issues in Computing, students may obtain the permission of an instructor in any upper-level (3000-4000) computing course to fulfill this requirement.
- Obtain your instructor's signature before submitting the form.
Waiver of School Policy
- Request an exception to School Policy due to extenuating circumstances.
